2010 Medical release-required for tournaments (print page 2 only)

Standard Team Financial Disclosure-This is the form for all parents to sign prior to joining the team. 2010-2011 program fees are included and team expenses are included as a sample. This form is in word and is customizable based on coaching, tournaments, level of play etc.
